Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill therapy services accurately per CMS expectations.
• Develop and implement treatment plans with clear goals.
• Screen residents to determine intervention needs and progress.
• Communicate findings to residents, families, and care team.
• Maintain compliance with CMS documentation and NetHealth EMR.
