Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Montgomery, AL. The PRN position supports the Physical Therapist in providing patient care and assisting with evaluations and treatment plans.

The role involves group and individual therapy, routine screenings, and collaboration with the care team to ensure high-quality resident outcomes. Licensure active and ongoing professional development are required.

Qualifications

• Associate's degree in physical therapy from an accredited program.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in the evaluation under the direction of the Physical Therapist.
• Assist in identifying target goals and treatment plans.
• Deliver individual and group treatment as outlined in the plan (e.g., strength, mobility, gait, ROM, modalities).
• Assist in selecting techniques/media and adapt as indicated by resident performance.
• Monitor resident response to intervention and consult with PT for modifications.
• Assist in establishing and modifying resident schedules with the PT and team.
• Adhere to schedules and modify as needed for treatment regimens.
• Train residents, family and staff in equipment use and applications.
• Participate in resident, family, and staff education.
• Ensure documentation per CMS expectations in NetHealth EMR.
