Zusammenfassung

Evolve Therapy Services, LLC is seeking a licensed Physical Therapist to join an in-house Skilled Nursing Facility in Selma, AL. This position offers full-time, part-time, or PRN scheduling and a patient-focused environment.

You will assess residents, develop and implement treatment plans, document in NetHealth EMR, and collaborate with an interdisciplinary team to ensure compliant, high-quality care. Minimum BS in PT, active license, and commitment to CE are required.

Qualifikationen

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
• Commitment to ongoing professional development.

Aufgaben

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures.
• Demonstrate judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Screen residents referred to physical therapy to determine need for intervention/treatment.
• Evaluate residents to obtain data necessary for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Develop treatment plans by establishing short and long-term goals and methods to achieve identified goals.
• Interpret and communicate evaluation findings, treatment plan and recommendations to residents, families/responsible parties, and interdisciplinary team members.
• Perform all other duties as assigned and as required to effectively discharge responsibilities.
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
