Job summary

Evolve Therapy Services seeks a licensed Physical Therapist Assistant to join an in-house Skilled Nursing Facility in Ahoskie, NC. You will work under the supervision of a Physical Therapist to deliver patient-centered therapy services and carry out treatment plans.

You will screen residents, assist in evaluations, and conduct both individual and group sessions, aiming to improve mobility, strength, balance and function. Documentation in NetHealth EMR is required.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services under the supervision of a PT.
• Screen residents at regular intervals to determine need for intervention.
• Assist in evaluations and treatment planning with the PT.
• Conduct individual and group treatment per treatment plan.
• Monitor resident response and modify interventions with PT input.
• Educate residents, families, and staff on therapy use and safety.
• Document compliant, medically necessary CMS‑aligned records in NetHealth EMR.
