Job summary

Evolve Therapy Services in Durham, NC is hiring a Physical Therapist Assistant for an in-house Skilled Nursing Facility. Full-time, part-time, or PRN positions available.

You will assist in planning, developing, and directing physical therapy services under the supervision of a Physical Therapist to ensure high quality resident care. Responsibilities include screenings, treatment planning, both individual and group therapies, and CMS documentation in NetHealth EMR.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing professional development.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluation and development of treatment plans with the Physical Therapist.
• Conduct individual and group treatments focused on strength, mobility, balance, and gait.
• Document compliant and medically necessary CMS/demonstrated records in NetHealth EMR.
• Train residents, families and staff in equipment use and safety.
• Participate in resident, family, and staff education.
