Job summary

Evolve Therapy Services is seeking compassionate Physical Therapists for an in-house Skilled Nursing Facility in Ahoskie, NC. Roles include full-time, part-time, or PRN options to support resident care.

Responsibilities include planning, implementing, and coordinating PT services, documenting per CMS guidelines, and collaborating with an interdisciplinary team to develop individualized treatment plans.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Timely screening of residents referred to physical therapy to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Communicate evaluation findings and recommendations to residents, families, and interdisciplinary team.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR systems.
