Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist to provide services at an in-house Skilled Nursing Facility in Ahoskie, NC. This position can be full time, part time, or PRN to fit your schedule.

Responsibilities include evaluating residents, developing treatment plans, document in CMS-compliant EMR, and collaborating with the interdisciplinary team to ensure high-quality resident care.

Qualifications

• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.
• BS degree in Physical Therapy from an accredited program.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound clinical judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention or treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Communicate findings and recommendations to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth or equivalent EMR.
• Collaborate with the team to ensure delivery of quality resident care.
