Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist Assistant for an in-house Skilled Nursing Facility in Ahoskie, NC. The role supports a licensed Physical Therapist and focuses on delivering hands-on therapy to residents in a facility setting.

Full-time, part-time, or PRN positions available. Responsibilities include screenings, treatment planning, and patient-family education, with CMS-compliant documentation in NetHealth EMR.

Qualifications

• Associate's degree in physical therapy (required).
• Active PTA license in state(s) of practice (required).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist in establishing target goals and treatment plans.
• Conduct individual and group treatment as outlined in the plan, including strength, mobility, balance, gait, ROM, and modalities.
• Assist in selecting PT techniques/media and adapt as indicated.
• Monitor residents’ responses and consult PT for modifications.
• Help establish and modify resident schedules with PT and team.
• Adhere to department and resident schedules for treatment.
• Train residents, families, and staff in equipment use.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in therapy planning and follow-up.
• Appear on time and coordinate schedules for maximum productivity.
• Assist with Quality Assurance programs as required.
• Complete CMS-compliant documentation in NetHealth EMR.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license
