Job summary

Evolve Therapy Services in Wilmington, NC is seeking a licensed Physical Therapy Assistant to support an in-house skilled nursing facility. Under the supervision of a Physical Therapist, you will implement treatment plans, conduct screenings, and help residents improve mobility and function.

Responsibilities include group and individual therapy, documenting CMS-compliant notes in NetHealth EMR, coordinating with the care team, and participating in QA activities.

Qualifications

• Associate degree in physical therapy is required.
• Active Physical Therapist Assistant license is required in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist the Physical Therapist in evaluating and planning resident care with short- and long-term goals.
• Assist in developing and modifying individual treatment plans.
• Deliver both individual and group therapy sessions per treatment plans; address mobility, strength, balance, and ROM.
• Monitor residents' responses and report modifications to the Physical Therapist.
• Document and maintain CMS-compliant notes in NetHealth EMR; ensure timely reporting.
• Coordinate with the care team to establish resident schedules and optimize productivity.
• Participate in quality assurance activities as required.
• Educate residents, families, and staff on therapy techniques and safety.
