Job summary

Evolve Therapy Services in Durham, NC is seeking a dedicated Physical Therapist Assistant for an in-house Skilled Nursing Facility. You will work under the supervision of a Physical Therapist to deliver patient-centered therapy services and contribute to resident care goals.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in North Carolina.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the supervision of a Physical Therapist.
• Conduct screenings to determine need for intervention or treatment.
• Assist in evaluations, treatment planning, and goal setting with the Physical Therapist.
• Deliver individual and group therapy addressing strength, mobility, gait, balance, and ROM.
• Train residents, families, and staff in therapy techniques and equipment use.
• Document compliant and medically necessary care per CMS expectations in EMR.
• Maintain resident schedules and collaborate with the care team to maximize productivity.
