Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Altoona, PA. The position is full-time or PRN and involves providing therapy under the supervision of a Physical Therapist.

The role focuses on screenings, evaluations, goal setting, and delivering both individual and group treatments, with emphasis on mobility, strength, balance, and functional training. CMS documentation is required in NetHealth EMR.

Qualifications

• Associate's degree from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.
• Maintain licensure and certification through ongoing development.

Responsibilities

• Provide physical therapy services under supervision of a Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluations and development of goals and treatment plans.
• Conduct individual and group treatment as outlined in the plan.
• Assist in selecting techniques/media and adapt as indicated.
• Monitor responses to intervention and consult as needed.
• Coordinate resident schedules with the PT and team.
• Train residents, families, and staff in equipment use.
