Evolve Therapy Services, LLC in Altoona, PA is seeking a Physical Therapist for an in-house Skilled Nursing Facility. You will evaluate, plan, and deliver physical therapy to residents to support high-quality care and compliance with standards.

Responsibilities include documentation, CMS-compliant notes, and collaboration with an interdisciplinary team. A licensed PT with ongoing professional development is required.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to professional development and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill physical therapy services per CMS expectations.
• Evaluate residents and develop short and long-term treatment goals.
• Screen residents regularly to determine need for intervention or treatment.
• Develop treatment plans and communicate findings to residents and families.
• Interpret evaluation findings and collaborate with interdisciplinary team.
• Complete compliant, medically necessary documentation in NetHealth EMR.
• Adhere to productivity standards and company procedures.
