Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant to support an in-house Skilled Nursing Facility. This full‑time or PRN role, located in Altoona, PA, operates under the supervision of a Physical Therapist to provide both individual and group therapy focused on mobility and function.

Requirements include an Associate's degree in Physical Therapy and an active PTA license. Ongoing education and professional development are expected.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services under the supervision of a Physical Therapist.
• Conduct resident screenings to determine intervention needs.
• Assist in evaluation planning with the Physical Therapist and set goals.
• Help develop treatment plans and schedules with the team.
• Deliver individual and group treatment focused on mobility, strength, balance, and ROM.
• Assist in selecting techniques and modalities with supervision.
• Monitor resident response and adjust as directed by the Physical Therapist.
• Coordinate resident schedules with the team for efficient care.
• Educate residents, families, and staff on therapy concepts and equipment.
• Maintain compliant CMS documentation in NetHealth EMR.
