Evolve Therapy Services, LLC is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Clarion, PA. This PRN position supports licensed Physical Therapists and delivers direct patient care to residents under supervision.

Responsibilities include screenings, treatment planning, individual and group therapy, and documentation in NetHealth EMR. Candidates should hold an active PTA license and an associate degree, with ongoing education expected.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.
• Ongoing licensure and certification maintenance

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in the evaluation process under the direction of the Physical Therapist.
• Help in setting short-term and long-term goals and treatment plans.
• Deliver individual and group treatment sessions focusing on strength, mobility, balance, and gait.
• Assist in selecting techniques/media and adapt plans per resident performance.
• Monitor resident responses and consult with the Physical Therapist for modifications.
• Coordinate resident schedules with the team and maintain treatment adherence.
• Train residents, family and staff in equipment use and safety.
• Participate in education for residents, families, and staff.
• Ensure compliant and medically necessary documentation in NetHealth EMR
