Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant to join our in-house skilled nursing facility team in Altoona, PA. You will work under the supervision of a Physical Therapist to deliver resident-centered care.

Responsibilities include screenings, assisting in evaluations and treatment planning, implementing therapy protocols, documenting CMS-compliant notes in NetHealth EMR, and educating residents, families, and staff.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Documentation per CMS expectations in NetHealth EMR systems.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and development of short- and long-term goals and treatment plans.
• Assist in implementing treatment plans including strength, mobility, balance, gait, ROM and modalities.
• Document CMS-required notes in NetHealth EMR and ensure accuracy.
