Evolve Therapy Services, a therapy management organization, seeks a Physical Therapist for an in-house Skilled Nursing Facility in Altoona, PA. The role supports planning, organizing, and directing physical therapy services in compliance with applicable standards and CMS expectations.

The position offers Full Time, Part Time, or PRN options. Requires a BS in Physical Therapy and an active PT license in the state, with ongoing continuing education.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards, procedures and productivity guidelines.
• Document and bill for services per company procedures.
• Develop treatment plans with short and long-term goals and communicate findings to residents and families.
