Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Altoona, PA. The PTA will work under the supervision of a Physical Therapist to deliver patient-centered therapy services and contribute to treatment plans.

The role requires an active PTA license, an associate degree, and ongoing professional development. Full-time or PRN status is available with CMS-compliant documentation in NetHealth EMR.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/tatientment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist in establishing short- and long-term goals and treatment plans.
• Conduct individual and group treatments addressing strength, mobility, gait, balance, and ROM.
• Monitor residents’ responses and adjust as directed by the Physical Therapist.
• Assist in scheduling with the team and ensure treatments align with CMS expectations.
• Document compliant and medically necessary notes in NetHealth EMR per CMS guidelines.
