Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant to support in-house skilled nursing facility operations in Paterson, NJ. You will work under the guidance of a Physical Therapist to deliver patient-centered rehabilitation services and ensure quality resident care.

Responsibilities include screenings, treatment planning supports, and documentation in NetHealth EMR, with opportunities for growth and professional development in a collaborative team environment.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluations and treatment planning under the Physical Therapist.
• Assist in establishing and modifying resident schedules with the team.
• Document compliant and medically necessary care in NetHealth EMR.
