Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Trenton, NJ. You will work under the supervision of a Physical Therapist to provide resident care and support, including screenings and treatment plan implementation.

Responsibilities include conducting individual and group sessions focused on mobility, gait, ROM, and balance, while ensuring compliant documentation and collaboration with the care team.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Assist in planning, organising, developing and directing PT services.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and development of target goals and treatment plans.
• Conduct individual and group treatment including mobility, gait, ROM and modalities.
• Document CMS/NetHealth entries accurately and timely.
