Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Paterson, NJ. The role supports a licensed Physical Therapist to deliver patient-centered PT services to residents.

Responsibilities include screenings, evaluation assistance, goal-driven treatment planning, and both individual and group sessions focused on mobility, strength, balance, and ROM. APT license and ongoing CE are required.

Qualifications

• Must have an Associate's degree in physical therapy.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Documentation aligned with CMS expectations.

Responsibilities

• Provide PT services under the supervision of the Physical Therapist.
• Screen residents at regular intervals for intervention needs.
• Assist in evaluations and treatment planning with the PT.
• Develop resident-specific treatment plans with short-/long-term goals.
• Perform individual and group treatment including mobility, strength, gait, ROM, and modalities.
