Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Trenton, NJ. You will work under a Physical Therapist to plan and deliver resident-centered PT services and help ensure quality care in a team setting.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention.
• Assist the Physical Therapist in identifying short- and long-term goals and treatment plans.
• Conduct individual and group treatment as outlined in the plan, including mobility and gait training.
• Document compliant and medically necessary information in NetHealth EMR as CMS requires.
