Job summary

Evolve Therapy Services, LLC in Trenton, NJ, is seeking a Physical Therapy Assistant to support an in-house Skilled Nursing Facility. This full-time position focuses on delivering quality therapy under the supervision of a Physical Therapist.

You will perform screenings, participate in evaluation and treatment planning, and implement plans that include mobility, gait, ROM, and modalities, while maintaining CMS-compliant NetHealth EMR documentation and resident education.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.
• Ability to document CMS expectations in NetHealth EMR.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluating residents and developing treatment plans with the Physical Therapist.
• Perform group and individual treatment including mobility, ROM, gait, and modalities.
• Document compliant and medically necessary CMS records in NetHealth EMR.
• Educate residents, families, and staff on therapy programs.
