Job summary

Pleasant Springs Healthcare Center in Mount Pleasant, TX invites a dedicated Physical Therapist (PT) to join our in-house team. You will assess patients, design individualized treatment plans, and guide rehabilitation to improve mobility and quality of life.

You’ll work within an interdisciplinary team to ensure care aligns with the plan of care and regulatory standards. The role requires an active Texas PT license in good standing, with SNF or long-term care experience preferred.

Qualifications

• Active and valid Texas PT license in good standing.
• SNF or long-term care experience preferred.
• Solid knowledge of physical therapy practice standards.
• Familiarity with Net Health documentation systems preferred.
• Ability to manage patients with varying needs and personalities.
• Strong communication, collaboration, and clinical decision-making.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Ensure all treatments are delivered in accordance with the established plan of care.
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations.
• Comply with all applicable rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab.
