Job summary

Texomahealthcenter is seeking a dedicated Physical Therapist (PT) to join our in-house team. This full-time or PRN role focuses on improving residents' mobility and quality of life through personalized rehabilitation plans and evidence-based practices.

You will work within an interdisciplinary team, document thoroughly, and participate in ongoing clinical education. Texas licensure and experience in SNF settings are highly valued.

Qualifications

• Texas PT license in good standing with regulatory boards.
• SNF or long-term care experience preferred.
• Knowledge of physical therapy treatment practices.
• Experience with Net Health EDS preferred.
• Ability to manage patients with varying personalities and needs.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer PT treatments to relieve pain and restore function.
• Document thoroughly and timely in compliance with policies and regulations.
• Collaborate with the interdisciplinary team to ensure high-quality care.
• Attend required rehab department meetings.
