Job summary

Hireology is seeking a dedicated Physical Therapist (PT) to join our in-house team. The role focuses on assessing and improving residents' mobility, strength, and quality of life within a SNF setting.

You must hold an active Texas PT license and be able to document care, collaborate with an interdisciplinary team, and educate patients and families. New grads are welcome; we offer flexible schedules with full-time or PRN options.

Qualifications

• Active Texas Physical Therapist license required.
• Experience in SNF or long-term care preferred.
• Excellent communication and collaboration with the care team.
• Familiarity with Net Health Electronic Documentation System preferred.

Responsibilities

• Assess patients to identify functional limitations.
• Develop and adjust individualized treatment plans.
• Administer PT treatments per care plan.
• Document care thoroughly and timely.
• Collaborate with interdisciplinary team for quality care.
• Educate patients, families, and caregivers.
• Monitor progress and modify interventions as needed.
• Attend required Rehab Director meetings.
