Job summary

Hireology is seeking a dedicated Physical Therapist (PT) to join our in-house team. You will assess, plan, and implement patient-centered therapy to improve mobility and quality of life in residents.

The role emphasizes collaboration with an interdisciplinary team, thorough documentation, and adherence to regulatory standards. Texas licensure and familiarity with SNF settings are preferred, with competitive compensation and benefits.

Qualifications

• Active and valid Texas Physical Therapist license.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Experience with Net Health documentation system.

Responsibilities

• Consult with patients to assess physical condition and functional limitations.
• Evaluate and interpret assessments and test results.
• Develop individualized treatment plans using various therapy techniques.
• Create personalized rehabilitation programs tailored to patient goals.
• Administer medically prescribed physical therapy treatments to relieve pain and restore function.
• Monitor patient response and modify approaches in collaboration with the care team.
• Provide clinical support and education to patients, families, and caregivers.
• Complete documentation thoroughly and timely per company policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
