Job summary

Longmeadow Healthcare Center is seeking a dedicated Physical Therapist (PT) to join our in-house rehab team. You will assess residents' mobility, design individualized treatment plans, and guide recovery toward improved function and quality of life.

You will collaborate with an interdisciplinary team, document thoroughly, and ensure care aligns with each resident’s plan of care. Texas PT licensure is required; SNF experience is a plus.

Qualifications

• Active Texas PT license in good standing.
• SNF or long-term care experience preferred.
• Knowledge of current physical therapy treatment practices.
• Experience with Net Health EDS preferred.
• Strong communication and teamwork skills.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Ensure all treatments are delivered in accordance with the established plan of care.
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations.
• Comply with all applicable rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab.
