Job summary

Creative Solutions in Healthcare - East is seeking a dedicated Physical Therapist (PT) to join our in-house rehab team. You will provide person-centered care to improve residents’ mobility, strength, and quality of life in a friendly SNF setting in Sherman, TX.

You’ll consult with patients, develop individualized treatment plans, administer PT services, and collaborate with an interdisciplinary team. Texas license and SNF experience preferred; Net Health EHR experience is a plus.

Qualifications

• Licensed Physical Therapist in Texas (PT) in good standing.
• SNF or long-term care experience preferred.
• Knowledge of physical therapy practices and Net Health EHR.
• Ability to adapt to varying patient needs and personalities.

Responsibilities

• Consult with patients to assess their physical condition and limitations.
• Evaluate and interpret assessments and test results.
• Develop individualized treatment plans using various therapy techniques.
• Create rehabilitation programs tailored to patient goals.
• Administer PT treatments to relieve pain and restore function.
• Monitor and adjust treatments with the interdisciplinary team.
• Document thoroughly and timely per policies.
• Attend required meetings with the rehab director.
