Job summary

Texoma Healthcare Center in Texas is seeking a dedicated Physical Therapist (PT) to join our in-house team of professionals. You will provide person-centered care to help residents improve mobility, strength, and overall quality of life.

You will assess patients, interpret tests, develop individualized treatment plans, administer therapies, document care thoroughly, and collaborate with an interdisciplinary team to ensure high-quality patient outcomes.

Qualifications

• Active Texas Physical Therapist license required.
• In good standing with licensing boards.
• SNF or long-term care experience preferred.
• Strong communication, collaboration, and clinical decision-making skills.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to assess physical condition.
• Evaluate and interpret assessments and test results.
• Develop individualized treatment plans using various therapy techniques.
• Administer physical therapy treatments to relieve pain and restore function.
• Document care thoroughly and timely.
• Collaborate with interdisciplinary team to ensure high-quality care.
