Job summary

Creative Solutions in Healthcare - East seeks a dedicated Physical Therapist (PT) to join our in-house team, delivering person-centered care to residents and helping improve mobility, strength, and overall quality of life.

The successful candidate will hold an active Texas PT license, have SNF/long-term care experience preferred, collaborate with an interdisciplinary team, document thoroughly, and educate patients, families, and caregivers.

Qualifications

• Active and valid Texas Physical Therapist license required or equivalent.
• In good standing with licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Ensure all treatments are delivered in accordance with the established plan of care.
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations.
• Comply with all applicable rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab
