Qualifications

• Illinois-licensed Physical Therapist Assistant (or eligible)
• BLS certification within 6 months unless exempt by leadership
• Valid driver’s license and reliable transportation

Responsibilities

• Develops and maintains restorative relationships with patients across care continuum.
• Implement plans of care and monitor patient progress with supervising therapist.
• Participates in staff meetings and educational activities to support professional development.
• Coordinate with care team to ensure safe, high-quality, cost-effective care.
• Assist in discharge planning and post-discharge care coordination.
• Educate patients and families to promote independence and self-care.
