Job summary

Advocate Health is seeking a Licensed Physical Therapist to provide home health services in the Far North/Northeast Suburbs of Illinois. You will assess and treat patients in their homes, travel within the assigned geographic area, and collaborate with physicians as part of a larger clinical team.

The role requires IL licensure (or eligibility), at least 1 year PT clinical experience, and strong documentation skills. A $10,000 sign-on bonus is offered, payable after hire and at 6 months.

Qualifications

• Licensed Physical Therapist in Illinois or eligible for licensure.
• At least 1 year of PT clinical experience or equivalent affiliation.
• Proficient with home-based therapy equipment and assistive devices.
• Basic computer skills: Word, email, and scheduling/documentation systems.
• Strong written and verbal communication; good documentation.

Responsibilities

• Travel extensively within geographic boundaries and weather extremes.
• Work rotating weekends and holidays; adhere to schedule.
• Perform home-based PT treatments with safe patient handling; lift up to 35 lb with equipment.
• Assess, plan, implement, and evaluate PT programs in the home to improve self-care.
