Job summary

Advocate Health is seeking a Physical Therapist Assistant (PTA) for Home Health in the Chicago area. The role requires Illinois licensure (or eligibility), BLS certification within 6 months, and a valid driver’s license. Experience rendering home health care and strong documentation is essential.

A $5,000 sign-on bonus is available for external candidates. The PTA will collaborate with supervising therapists to implement individualized plans of care, travel within the Chicago South & Southwest

Qualifications

• Licensed Physical Therapist Assistant in Illinois or eligible for licensure.
• Basic Life Support (BLS) for Healthcare Providers certification within 6 months.
• Valid driver’s license.
• 2 years of clinical experience or demonstrable clinical affiliation experience.

Responsibilities

• Develops and maintains a restorative relationship across the healthcare continuum per Advocates MVP of Compassion.
• Adapt plan of care under supervision of the therapist; request reassessment as needed.
• Provide care that meets or exceeds patient and family expectations; document thoroughly for reimbursement and compliance.
• Promote patient/family knowledge to safely manage self-care and health conditions.
