CenterWell Home Health in Illinois is seeking a Home Health Physical Therapist Assistant to deliver in-home PT services, follow the Physical Therapist's plan, and help patients regain strength, mobility, and independence. The role emphasizes teamwork, documentation, and patient safety during home visits.

Ideal candidates have a current PT license, valid driver's license, CPR certification, and experience with geriatric care.

Formación

• Current state license to practice Physical Therapy.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric physical, emotional, social, and biological changes.

Responsabilidades

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapies and modalities as prescribed.
• Document client records and submit clinical documentation on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply shortages, or staff or client changes.
• Apply infection control and universal precautions in patient care.
