Job summary

Advocate Health seeks a Licensed Physical Therapist for Home Health in Illinois. You’ll deliver patient-centered care in patients’ homes, collaborate with an interdisciplinary team, and travel within the territory to support mobility and function.

This is a full-time role, Mon–Fri, 8:00 AM–5:00 PM, with rotating weekends and a sign-on bonus for eligible external candidates. Illinois licensure (or eligibility), 1 year of clinical experience, and proficient documentation in electronic systems are

Qualifications

• Licensed Physical Therapist in Illinois or eligible for licensure.
• Minimum 1 year of clinical experience (or strong clinical affiliation).
• Proficient in documentation using electronic systems.

Responsibilities

• Evaluate and implement individualized treatment plans.
• Monitor progress and adjust plans for outcomes.
• Provide timely, compliant documentation for care and reimbursement.
• Educate patients and families on plans and safety.
