Qualifications

Responsibilities

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide in-home therapy services focused on improving physical and cognitive functioning.
• Monitor, document, and report patient responses to treatment and changes in condition.
• Educate patients and caregivers on therapeutic exercises, safety, and use of adaptive equipment.
• Supervise Physical Therapy Assistants and, when appropriate, Certified Nursing Assistants.
• Complete required documentation, prepare clinical reports, and participate in team and agency development activities.

Required Qualifications

Schedule & Travel

Monday–Friday | Day shift (start and end times vary based on patient needs)

One weekend every six weeks (Saturday & Sunday, scheduled visits only)

Two holidays per year (one summer, one winter on a rotating basis)

No on-call requirements

Local travel to residential homes
