Job summary

Advocate Health is seeking a Physical Therapy support role within Home Health Care to provide high-quality patient-focused care across the Joliet area. Qualified candidates will have an Illinois PTA license or eligibility, BLS certification, a valid driver’s license, and at least 2 years of clinical experience.

Qualifications

• Illinois PTA license (or eligibility)
• BLS for Healthcare Providers certification within 6 months
• Valid driver’s license
• 2 years of clinical experience or demonstrated strong affiliation experience

Responsibilities

• Develops and maintains restorative patient relationships across the care continuum with compassionate care.
• Assess patient readiness and involve family in treatment; escalate to supervisor as needed.
• Document thoroughly to support reimbursement and regulatory requirements.
• Promote patient education and self-care for safety and infection control.
• Coordinate with supervising therapist and interdisciplinary team for care plan updates.
