Job summary

Advocate Health Care is seeking a Physical Therapist Assistant (PTA) for Home Health in the Chicago South & Southwest Service Area. The role offers a 40-hour workweek, Monday through Friday, 8:00 AM to 5:00 PM, with a $5,000 sign-on bonus for external candidates.

Licensure in Illinois and BLS are required, with meaningful travel to patient homes and coordination with supervising therapists. The position emphasizes relationship-based care, adherence to MVP of Compassion, and ongoing professional

Qualifications

Licensed in Illinois (or eligible for licensure).
BLS for Healthcare Providers certification required within 6 months unless not required by department leader.
2 years of clinical experience or demonstration of strong spectrum of clinical affiliation experience.

Responsibilities

Develops and maintains restorative relationships with patients throughout the healthcare continuum and supports the plan of care under supervising therapist.
Utilizes evidence-based interventions and documents thoroughly to support reimbursement and regulatory requirements.
Participates in team meetings and educational activities to enhance development and support organizational initiatives.
Coordinates care with the interdisciplinary team and supports discharge planning and post-discharge care coordination.
