Job summary

CenterWell Home Health is hiring a Speech Language Pathologist to deliver home-based speech-language pathology services. You will evaluate patients, develop treatment plans and work within care teams to improve communication, voice and swallowing disorders.

A state license, ASHA CCC, CPR and reliable transportation are required. In this role you will document assessments, participate in care coordination, and provide training to patients and families while adhering to confidentiality standards.

Qualifications

Certification of Clinical Competence in Speech-Language Pathology (CCC-SLP) or Audiology (CCC-A) per ASHA.
Minimum six months experience as a speech therapist/speech-language pathologist.
Current and unrestricted license.
CPR certification and valid driver’s license with reliable transportation.

Responsibilities

Evaluate and provide speech-language pathology services in patient homes or facilities.
Develop and review Plan of Treatment and Plan of Care.
Assess and treat disorders of speech, voice, language, hearing and swallowing.
Administer and interpret diagnostic tests and therapeutic treatments.
Record changes and report to supervisor/physician.
Provide instruction on alternative communication methods when appropriate.
Maintain documentation and submit reports per company policy.
Participate in care coordination and discharge planning.
