Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in their homes or facilities. You will develop and review plans of treatment and care, assess speech, voice, language and swallowing disorders, and provide patient and caregiver training.

Strong documentation and coordination with care teams are essential. Requires ASHA certification, active license, CPR, and drivers license with reliable transportation; home health experience preferred.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology or Audiology per ASHA.
• Minimum six months experience as a speech therapist / speech-language pathologist.
• Current and unrestricted license.
• CPR certification and valid driver's license with reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapies.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction on alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff.
• Maintain required documentation and write reports.
