Job summary

CenterWell Home Health is seeking a Speech-Language Pathologist to deliver in-home therapy, evaluate and treat speech, language, voice and swallowing disorders, and support patients’ independence. You will collaborate on treatment plans, document progress, and assist in care coordination.

The role requires ASHA CCC-SLP/CCC-A, a minimum six months of home health experience, current license and CPR, plus a valid driver’s license and auto insurance for patient visits.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA) required.
• Minimum six months of experience as a speech-language pathologist.
• Current and unrestricted professional license.
• Current CPR certification.
• Valid driver's license, automobile insurance, and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients at home or facility.
• Participate in development and periodic review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic treatments.
• Observe, record and report changes to supervisor/physician.
• Provide instruction on alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain required documentation.
• Participate in care coordination and discharge planning.
