Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in their homes or facilities. You will develop and review treatment plans, assess speech, language, voice and swallowing disorders and provide guidance to patients and families.

Responsibilities include documentation, care coordination, and discharge planning, with emphasis on rehabilitation and functional improvement to help patients regain independence.

Qualifications

• Certification by ASHA (CCC-SLP) or equivalent.
• Minimum six months of experience as a Speech-Language Pathologist.
• Current CPR certification and active state license.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Develop and periodically review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic applications.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction to patients and families in communication strategies.
• Maintain documentation and submit reports as required by policy.
• Participate in care coordination and discharge planning.
