Job summary

CenterWell Home Health seeks a licensed Speech-Language Pathologist to provide in-home or facility-based therapy, evaluating and treating speech, language, voice and swallowing disorders. You will develop and review treatment plans, document progress, and collaborate with care teams to optimize patient outcomes.

The role requires ASHA certification or state licensure, CPR, and a valid driver’s license, with experience in home health valued.

Qualifications

• ASHA CCC or state licensure required.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.

Responsibilities

• Evaluate, direct and provide speech-language pathology services to patients in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic applications.
• Observe and report changes in patient condition to supervisor or physician.
• Provide instruction to patients and families on communication methods.
• Maintain and submit required documentation.
