Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in home settings or facilities. You will develop and review care plans, administer assessments, and coordinate with caregivers and physicians to promote rehabilitation and independence.

Required ASHA CCC, a minimum of six months experience, proper licensure, CPR, and a valid driver’s license. The role offers base pay with comprehensive benefits and a strong emphasis on patient-centered care.

Qualifications

• Must have ASHA Certification of Clinical Competence in Speech-Language Pathology or Audiology
• Minimum six months experience as a speech-language pathologist
• Current and unrestricted license in practicing state
• Current CPR certification
• Valid driver's license, auto insurance, and reliable transportation

Responsibilities

• Evaluate, direct, and provide speech-language pathology services in home or facility
• Develop and review Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing, and swallowing
• Administer and interpret diagnostic tests and therapeutic applications
• Record changes in patients’ condition and communicate with supervisor/physician
• Provide training in alternative communication systems when appropriate
• Counsel patients, families, and healthcare staff
• Submit documentation and progress reports as required
• Participate in care coordination and discharge planning
• Maintain confidentiality and comply with company policies
