Job summary

CenterWell Home Health invites a licensed Speech-Language Pathologist to deliver in-home therapy, focusing on rehabilitation and functional improvement for patients. You will evaluate, treat, and document progress while collaborating with the care team to adjust plans of treatment.

The role requires ASHA CCC-SLP eligibility, at least six months of pediatric or adult SLP experience, a valid license and CPR certification, plus reliable transportation for home visits.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech-Language Pathology or Audiology granted by ASHA.
• Minimum six months experience as a speech therapist/speech-language pathologist.
• Current and unrestricted license; CPR certification and valid driver’s license with insurance and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech-language pathology service to patients in their home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; administer and interpret diagnostic tests.
• Observe, record and report changes in patient condition to supervisor/physician; maintain documentation per policy.
• Provide instruction for alternative communication systems and counsel patients, families, and staff.
• Participate in care coordination activities and discharge planning; ensure confidentiality and proper sharing of information.
