Job summary

CenterWell Home Health invites a Speech Language Pathologist to evaluate and treat patients in their homes or facilities, shaping and updating the Plan of Treatment and Plan of Care. The role focuses on rehabilitation of speech, language, voice and swallowing, with regular testing, documentation and care coordination.

This position requires ASHA CCC or equivalent certification, an active license, CPR, and a valid driver’s license to support travel for home visits.

Qualifications

• Certification of Clinical Competence in Speech-Language Pathology or Audiology (ASHA) required.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license; CPR certification; valid driver’s license and transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in the home or facility.
• Develop and periodically review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; administer diagnostic tests and therapeutic treatments.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff; maintain documentation including evaluations, visits, care plans and progress reports.
• Participate in care coordination and discharge planning; share information following policy and confidentiality standards.
