Job summary

Scylla Solutions LLC is seeking a Speech Language Pathologist to provide outpatient rehabilitation in an in-home setting for older adults. Services may occur in private homes or in assisted living/independent living facilities, enabling therapy in the patient's functional environment.

Responsibilities include supervising CFYs and staff, adhering to schedules and documentation, and collaborating with the rehab team to tailor treatment plans.

Qualifications

• Must have valid Speech Language Pathologist license in state(s) of practice or license eligibility.
• Must possess or be eligible for ASHA Certificate of Clinical Competence or ASHA Clinical Fellow eligibility.
• Must have a valid driver's license or reliable transportation to patient residences.

Responsibilities

• Supervise CFYs, students and support personnel per state licensure requirements.
• Adhere to patient treatment schedules and complete clinical/billing documentation.
• Attend and contribute to patient care conferences and related meetings.
• Make referrals to other disciplines to address patient needs.
• Consult with rehabilitation team and physician to select appropriate therapy.
• Adapt plan of care to individual patient needs and goals.
• Maintain a safe environment and remove hazards; order supplies as needed.
• Coordinate care in home/community with interdisciplinary team and referring physician.
