Job summary

Scylla Solutions is seeking a Speech Language Pathologist to provide outpatient rehabilitation services in in-home settings for older adults in New Hampshire. The role involves evaluating and treating patients in their homes or in assisted living facilities, coordinating with the rehabilitation team, and promoting safe, effective care with flexible scheduling.

Qualifications

• Must hold a valid Speech Language Pathologist license in the practicing state or be eligible for licensure.
• Must possess or be eligible for ASHA CCC or participate as ASHA Clinical Fellow.
• Must have a valid driver’s license or reliable transportation to patient residences.

Responsibilities

• Supervise CFYs, temporary licensees, students and support personnel per state licensure and standards.
• Adhere to department and patient treatment schedules; complete clinical and billing documentation.
• Attend patient care and staffing conferences; contribute to related meetings.
• Refer to other disciplines to address comprehensive patient needs; coordinate with rehabilitation team.
• Adapt plan of care to individual patient needs; select appropriate therapeutic regimens.
• Promote safe environment; order supplies and equipment; maintain and repair equipment as needed.
• Coordinate home and community care with interdisciplinary team and referring physician.
