Job summary

Beyond Home Health Care is an approved NJ DDD provider delivering in-home speech-language services across New Jersey. We seek a licensed Speech-Language Pathologist to evaluate and treat adults with developmental disabilities in-home and in the community, following NJ DDD guidelines and person-centered planning.

Responsibilities include comprehensive evaluations, individualized plans, caregiver training, and interdisciplinary collaboration.

Qualifications

• Current New Jersey license as a Speech-Language Pathologist.
• ASHA CCC-SLP certification required; CFs considered with supervision.
• Master's degree in Speech-Language Pathology from an accredited program.
• Minimum of one year of clinical experience preferred; CFs and new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver’s license, and current auto insurance.
• Ability to pass DDD background checks including CRoff.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals.
• Deliver one-on-one therapy services in the participant's home and community settings.
• Document services per DDD standards, including session notes, progress notes, and outcomes.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team.
• Provide caregiver training and home programs to reinforce goals.
• Adhere to DDD policies, HIPAA, and related procedures.
• Maintain billing records consistent with DDD rates and authorizations.
• Complete required DDD trainings (Pre-Service, Danielle's Law, etc.).
