Qualifications

• Must have valid SLP license in practice state.
• Must have reliable transportation to patient residences.

Responsibilities

• Coordinate care with the rehab team and physician to tailor treatment plans.
• Assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders.
• Ensure documentation and billing compliance and participate in team meetings.
